Employers of any size — from 3 to 400+ employees — can sponsor memberships at the same per-person rates: $110 employee, $100 spouse, $75–$150 for children.

Specialty labs, certain advanced procedures (such as biopsies and vasectomies), and some imaging require additional cost — though we coordinate competitive wholesale options whenever possible. Pricing for these services is upfront and transparent; contact us directly for specifics.
No hidden fees. Membership requires a 6-month initial commitment; after that, cancellation requires 30 days’ notice. Re-enrolling within 18 months of cancellation incurs a $400 fee.
