What is Direct Primary Care, or DPC?

Direct Primary Care (DPC) is a membership model where comprehensive primary care services are provided in an innovative way for an affordable, monthly fee — without billing insurance for visits.

How much does it cost?

The monthly fee is $110 for individuals or employees, $100 for spouses/partners, $75 for one child/dependent age 17 and under, and $150 for two or more children/dependents age 17 and under. See our Pricing page for full details.

How is DPC better or different from traditional primary care?

Our incentives favor your health over visit volume. Appointments run 30–60 minutes with minimal wait times. We focus on proactive prevention rather than reactive treatment, offer predictable costs with no surprise bills, deliver care flexibly (in person, phone, text, or video), and give each member individualized attention instead of insurance-driven checklists.

Why would I pay an "extra" monthly fee for primary care? Is this concierge medicine?

The membership fee is best thought of as a more affordable and predictable way to access better primary care. DPC is not concierge medicine — it’s primary care that provides a more personalized healthcare experience at a price designed to be affordable.

What is included in the monthly fee?

The monthly fee covers comprehensive primary care for the whole family, including wellness, prevention, health coaching, chronic disease management, acute visits, and care for the occasional sick day. Members receive unlimited visits, point-of-care labs, in-office procedures, specialist e-consults, pharmacy discounts, and wholesale imaging access.

How much do non-included services cost?

For services not included in your membership, such as in-office procedures like joint injections and skin biopsies, our pricing is upfront and transparent. Contact us directly for specific pricing inquiries.

Are there any hidden fees or commitments? What if I decide to cancel?

There are no hidden fees — we aim for fully transparent pricing. Membership requires a 6-month initial commitment; after that, cancellation requires 30 days’ notice. Re-enrolling within 18 months of cancellation incurs a $400 fee.

Who can join SevenHeaven Care? How do I sign up and book an appointment?

Children and adults of any age can join. Employers of any size can sign up employees and their families. We are currently waitlist-only for physicians at both locations — email us to join the waitlist, and once enrolled you can book via phone, text, or our member app.

Does SevenHeaven Care partner with businesses and employers?

Yes. We partner with employers ranging from 3 to 400+ employees, offering meaningful cost savings and improved retention. We’re part of a national DPC network that supports multi-location businesses across states.

Can children become members? Do you offer women’s health and OB care?

As family physicians, we’re trained to care for the whole family. Our services include pediatric care, women’s health, and OB care, with delivery privileges at NewYork-Presbyterian Hospital.

What if I am healthy and rarely go to the doctor?

We fully support extremely healthy members, those managing multiple chronic conditions, and everyone in between. Our focus includes preventive care, health optimization, and access for unexpected illness whenever it arises.

What existing medical conditions can you care for? What about mental health?

Our training allows us to care for a full range of chronic conditions, including diabetes, hypertension, high cholesterol, heart disease, thyroid disease, allergies, autoimmune conditions, asthma, COPD, obesity, insomnia, anemia, kidney issues, arthritis, and complications of tobacco, alcohol, and other substance use, among others. Mental health services include management of anxiety, depression, bipolar disorder, OCD, and ADHD.

How do you feel about alternative and integrative approaches to health care?

We deliver evidence-informed care, seeking robust scientific evidence for everything we recommend while adapting it to your personal values. Our providers emphasize whole-person care and lifestyle medicine.

What is health coaching?

A health coach is a professional with expertise in health behavior and lifestyle change, with the skills and time to help you discover your own direction, motivation, and support in reaching your personal health goals.

Do you offer any services for non-members? Do you take walk-ins?

We offer appointments to non-members only for vasectomy procedures with Dr. Marcus Whitfield. All appointments, member or not, require scheduling by phone, text, or online.

What are your office hours? What about after hours and weekends?

Our Hoboken office is open Monday–Friday, 9 a.m.–5 p.m. Our Manhattan office is open Monday, Tuesday, Thursday, and Friday, 9 a.m.–5 p.m. Physicians remain available by phone for urgent issues outside office hours; home visits are available at additional cost.

Who are the team members at SevenHeaven Care?

How much time do I get with providers at appointments?

Our shortest appointment is 30 minutes; annual or comprehensive appointments run 60 minutes.

Do you offer memberships outside of New Jersey and New York? What if I need care while traveling?

Our physicians are licensed in New Jersey and New York and cannot practice medicine in other states. That said, virtual guidance is available from anywhere, and we can help you locate a DPC practice near you through our national network if you need in-person care while traveling.

Is a SevenHeaven Care membership insurance, or a substitute for health insurance?

No — we are neither health insurance nor an alternative to health insurance. We recommend members carry a separate health insurance or medical cost-sharing plan.

Can SevenHeaven Care bill my insurance?

We do not bill your insurance for our services and do not contract with any public or private insurance plans. Itemized invoices are available for non-included services if you’d like to submit a claim on your own.

Can I still use insurance elsewhere? What about specialized or inpatient care?

Yes. Members use their insurance for referrals to specialists, imaging, and surgery. We help navigate those options and provide competitive cash-pricing alternatives when available.

Do I need to keep health insurance?

To ensure your access extends beyond our walls, particularly for unexpected and expensive events, we recommend everyone have and keep health insurance. Our services remain the same regardless of your insurance status.
